Tournament application form

Churwell Lions would like to invite you to take part in the 2008 Tournament. Entries will be on a first come first served basis, so early return is advised to avoid disappointment.

Date: -Saturday 19th July 2008
Venue: -Bruntcliffe High School, Bruntcliffe Lane, Morley, Leeds. LS27 OLZ
Entry Fee: -£20 per team
Closing Date for entries: - 30th June 2008

Morning: -
Age groups 7’s, 9’s, 11’s, 13’s and 15’s.
Under 7’s teams will consist of 5 Players plus 2 subs.
Under 9’s, 11’s, 13’s and 15’s teams will consist of 6 Players plus 2 subs.
Registration 9:15 am, first game commences at 9:45 am

Afternoon: -
Age groups 8’s, 10’s, 12’s and 14’s.
Under 8’s teams will consist of 5 Players plus 2 subs.
Under 10’s, 12’s and 14’s teams will consist of 6 Players plus 2 subs.
Registration 1:30 pm, first game commences at 2:00 pm.

Could any teams who would like to take part, please complete the entry form below stating the number of teams they wish to enter and return it to the club treasurer at the above address, along with a cheque made payable to ‘Churwell Lions J.F.C’.
